Understanding 245D Waivered Services

The 245D Waiver is a Minnesota-specific regulation that allows home and community-based services for individuals with disabilities and older adults. Allied Home Healthcare is a licensed Basic 245D provider offering services including:

  • Individualized Home Supports Without Training
  • Individualized Home Supports With Training
  • Respite Services
  • Homemaker Services
  • Night Supervision

Available Waivers

These services are available to individuals eligible under:

  • Brain Injury (BI) Waiver
  • Community Access for Disability Inclusion (CADI) Waiver
  • Community Alternative Care (CAC) Waiver
  • Developmental Disabilities (DD) Waiver

We provide 24-hour emergency services through our trained crisis response team. If you’re facing an urgent situation related to care, please reach out immediately.
